I accept First Choice Healthcare, Group Health Options, L&I (work-related claims) and PIP (auto injury claims).
For other insurance companies you may be able to see me if you have out-of-network benefits, however you would be responsible for all costs that your insurance company does not cover. Flexible Spending Accounts and Health Savings Accounts may be used if massage therapy is covered by your employee policy.
If you are using any insurance a referral or prescription is required from your doctor prior to starting treatment. The referral or prescription must contain your diagnosis code(s), the start date for treatment, the frequency and duration of treatment, and the doctor’s name and signature. If I am in network with your insurance I will send the bill for your treatment directly to the insurance company. All co-pays are due at the time of service. If your insurance company denies your claim(s) you are financially responsible for the sessions, the charge will be the same as the fee structure listed above. Please contact your insurance company to get details on your massage benefits prior to your appointment.
Methods of Payment:
I accept cash, check, Visa, MasterCard, American Express, and Discover. Payment is due at the time services are provided. I do have gift certificates available for purchase.
